QuestionnaireBefore any offer, we run the pre-hire checks this role legally requires: screening against the HHS-OIG List of Excluded Individuals/Entities (LEIE), SAM.gov, and state Medicaid exclusion lists, plus primary-source verification of every license and credential with the issuing board. In this step you confirm your credential details and authorize those checks. Because the OIG updates the LEIE monthly, exclusion screening continues after hire.
